The options here are fairly unpleasant, both, in what they entail and in their after-effects. Surgery can be an option only rarely, such as in the case of someone who's severely obese and has other life-threatening health problems.

In gastroplasty, for instance, the surgeon uses staples or sutures across the upper one-third of the stomach to create a pouch with a narrow length of channel to connect the stomach to the small intestine.

By reducing the capacity of the stomach, procedures such as this one force a smaller intake of food. The narrow channel created in gastroplasty also effectively slows down the rate at which food transits into the small intestine, thus keeping the stomach full for a longer period.

Do surgical measures work? Several of those who have undergone such surgery have lost from 30 to 50 per cent of their excess fat over a period of a year and a half to two years; however, few people have reached their ideal weight by this means. What's more, most of those who've had the surgery reversed have regained all the weight they'd lost. Common side-effects of such surgical procedures are nausea, vomiting and a feeling of abdominal fullness and discomfort.

Surgery has been generally resorted to only in those who are grossly obese (about 40 per cent over the ideal weight), and in whom other forms of weight control have failed.

(Liposuction, the sucking out of fat cells, is not a treatment for obesity since there's a safe limit to the amount of fat that can be vacuumed out at one sitting — not more than two and a half kilos. Liposuction, therefore, is more a spot-reduction method to tackle stubborn, localized deposits of fat — saddle bags, love handles, thunder thighs — that have resisted all efforts at dieting and exercise. So is lipectomy, which involves cutting off both excess fat and skin in specific areas such as the abdomen or the upper arms.
